Ordering a Transcript by Mail or In Person

Note: Transcript requests cannot be e-mailed or taken by telephone. Transcripts cannot be faxed.

 To request an Official Transcript by mail, please use the Click Here to Download Transcript Request form or include the following information:

• Name under which you were enrolled at CIUS Ontario
• Student ID number (if available)
• School or division in which you were enrolled.
• Dates of attendance
• Certification (s) earned and date(s) awarded, if any
• Complete address to which transcript is to be sent
• Credit card information if paying by credit card (include credit card number, expiration date, and billing  address) 
• PHONE NUMBER OR E-MAIL ADDRESS (in case we encounter a problem & need to contact you) 
• YOUR SIGNATURE AND THE DATE (we will not process a request without your signature)
